Notice of Privacy Practices
Effective Date: August 20, 2026. This Notice describes how medical and health information about you may be used and disclosed by Therapy With MJ, LLC ("the Practice," "we," "us") and how you can access that information. Please review it carefully. This Notice applies to protected health information ("PHI") created or received by the Practice in connection with therapy services. Life coaching and supervision services are not medical treatment and are not governed by HIPAA, but the Practice extends the same confidentiality principles to those relationships wherever reasonably possible.
The Practice is required by law to maintain the privacy of PHI, to provide individuals with this Notice describing its legal duties and privacy practices, and to abide by the terms of the Notice currently in effect.
Uses and Disclosures of Health Information
The Practice may use and disclose your PHI for the following purposes without your written authorization:
- Treatment — to provide, coordinate, or manage your care, including consultation with other treating providers when appropriate.
- Payment — to bill and collect payment for services, including verifying insurance coverage or benefits when applicable.
- Health Care Operations — for activities such as quality assessment, supervision, training, and business management.
- Required by Law — when disclosure is required by federal, state, or local law, including mandatory reporting of suspected abuse or neglect of a child, elder, or dependent adult.
- Health or Safety — to prevent or lessen a serious and imminent threat to your health or safety or that of another identifiable person, consistent with applicable law and professional ethical standards.
- Public Health, Legal, and Regulatory Purposes — such as court or administrative orders, subpoenas, or as otherwise required by state law.
- Business Associates — with vendors (for example, secure scheduling, billing, or telehealth platforms) who agree in writing to protect your information consistent with HIPAA.
Uses that require your written authorization. Psychotherapy notes (the Practice's private process notes, kept separate from the clinical record), marketing communications, and any sale of PHI require your specific written authorization, which you may revoke at any time in writing except to the extent action has already been taken in reliance on it. Any other use or disclosure not described in this Notice will only be made with your written authorization.
Your Rights
Subject to certain conditions and limitations under applicable law, you have the right to:
- Request restrictions on certain uses and disclosures of your PHI. The Practice is not required to agree to a requested restriction, except where you paid out of pocket in full for a service and ask that it not be disclosed to your health plan.
- Request confidential communications — for example, asking to be contacted only at a certain phone number or email address.
- Inspect and obtain a copy of your health record, with limited exceptions (such as psychotherapy notes).
- Request an amendment to your health record if you believe information is incorrect or incomplete.
- Receive an accounting of certain disclosures of your PHI made by the Practice, other than for treatment, payment, operations, or a few other exceptions.
- Receive a paper copy of this Notice at any time, even if you agreed to receive it electronically.
- File a complaint if you believe your privacy rights have been violated (see Complaints below), without fear of retaliation.

Our Responsibilities
The Practice is required by law to maintain the privacy and security of your PHI, notify you if a breach occurs that may have compromised the privacy or security of your information, follow the duties and privacy practices described in this Notice, and not use or share your information other than as described here unless you tell us we can in writing (and you may revoke that permission in writing at any time).
The Practice reserves the right to change the terms of this Notice and to make the revised Notice effective for PHI it already has as well as any information it receives in the future. An updated Notice will be made available on this website and provided upon request.
Minors and Parental Access
When services are provided to a minor, the rights of parents or guardians to access records and consent to treatment vary by state and by the circumstances of care. The Practice follows the applicable law of the state in which services are rendered (currently Texas, Virginia, North Carolina, and South Carolina) and will discuss any state-specific limits on confidentiality with a parent or guardian, and with the minor client as clinically appropriate, before treatment begins.
Telehealth Services
Sessions may be conducted virtually using a secure, encrypted platform intended to protect the confidentiality of your information. You are responsible for participating from a private location where you will not be overheard and for the security of your own device and internet connection. As with any electronic communication, no telehealth platform can guarantee absolute security; the Practice will let you know if it becomes aware of a security concern affecting a platform it uses.
Security and Confidentiality of Records
The Practice uses reasonable administrative, physical, and technical safeguards designed to protect your personal and health information, including secure record-keeping systems and limited access to client information. No electronic system can guarantee absolute security. Please do not send sensitive clinical information, and never use the general website contact form for urgent, emergency, or highly sensitive matters — use your designated secure client portal, or call the Practice directly, instead.
Website & Contact Form Privacy
Separately from the clinical record described above, this website collects limited information you choose to submit through the Contact form — your name, email address, phone number (if provided), the topic you select, and your message. That information is used solely to respond to your inquiry and, unless you tell us otherwise, to add you as a contact in the Practice's client relationship system so we can follow up. It is not sold, rented, or shared with third parties for their own marketing purposes.
The website itself runs on Wix's hosting platform, which may set essential cookies needed for the site to function and to protect against spam and abuse. The Practice does not currently use third-party advertising trackers on this site. If that changes, this policy will be updated accordingly.
If you are a resident of a state with its own consumer privacy law (including Virginia's Consumer Data Protection Act), you may have additional rights regarding personal information collected through this website; contact the Practice to make a request.
Complaints
If you believe your privacy rights have been violated, you may file a complaint with the Practice using the contact information below, or with the U.S. Department of Health and Human Services, Office for Civil Rights, by visiting www.hhs.gov/hipaa or calling 1-800-368-1019. You will not be retaliated against for filing a complaint.
